By submitting this form, you consent to and acknowledge that the Pharmacy Guild of Australia (ABN 84 519 669 143) (the Guild) will collect your name, pharmacy name, email address and Modified Monash Model (MMM) number (Personal Information) as part of the I Love Rural Pharmacy Video Competition (the Video Competition).
Please be aware that any video you enter in the Video Competition may include both your Personal Information and the Personal Information of other people who are featured in the video. In entering the competition, you confirm that you consent, and that you have obtained the consent of those other people, to a use of their personal information as outlined in the Terms and Conditions (LINK) and this notice.
The Personal Information you provide will be used solely for purposes directly related to the Video Competition. This includes publicly identifying you as an entrant, contacting you if we need to clarify or discuss your video submission, and notifying and announcing you if you are selected as a winner (the Services). Without this information, we would be unable to provide you with the Services. In addition, as indicated in our Terms and Conditions may we use the video for a number of purposes related to the Guild’s functions. We will not otherwise use your Personal Information (or the personal information of any other persons featured in the video) for any other purpose unrelated to the Video Competition without contacting you.
