Authority to contact accountant

As a member of The Pharmacy Guild of Australia (Guild), and the owner (you and your) of the pharmacy business or pharmacy businesses identified above (each your pharmacy), you acknowledge and confirm that:

1. you understand that the Guild Digest is a periodical publication prepared by the Guild that provides a snapshot of community pharmacy operations in the previous financial year and is also used for negotiations between the Guild and the Government and third parties for the benefit of the Guild’s members;
2. you understand that the Guild is requesting owners of community pharmacies to voluntarily provide financial information in relation to their pharmacy businesses for the financial year ending 30 June 2023 for inclusion in the 2024 Guild Digest (owners that participate and their pharmacy businesses are participating pharmacies);
3. if there is more than one owner, or if the owner is a body corporate, this authorisation is given for and on behalf of each of the owners, and in the case of a body corporate, is authorised by its shareholders and directors;
4. by completing the Accountant and Your Pharmacy fields below and submitting this form, you authorise the Guild to contact your accountant to provide the Guild with the profit and loss statement and balance sheet for each of your pharmacies for the financial year ending 30 June 2023 (financial data);
5. you confirm that you own the financial data and there is no restriction on your right to share the financial data with the Guild for any purpose that is described as an approved purpose in the data collection statement set out below;
6. you agree to the use and de-identification of the financial data on the basis provided in the data collection statement set out below; and
7. you agree to the use of the project data for the approved purpose, and on the basis as provided in the data collection statement set out below.
